A31 Group of Practices
As a result of changes in the GP Contract, practices are now working together in groups – typically covering between 30 to 50 thousand patients. We are a member of the A31 Group of Practices
Primary Care Networks (PCNs) build upon the core of previous primary care services and allow greater provision of proactive, personalised, co-ordinated and better integrated health and social care.
Primary Care Networks are the foundation for future collaborations with other providers such as Community Trusts where appropriate, and will work together to bring specific packages of care to improve the health of patients.
